Immunization Requirements

The immunizations below are required before your child can be admitted to school.

We are required to remove students from school who are not fully immunized or have incomplete records.

Immunization Requirements
DTP/DT/DTAP 4 doses are required. A 5th dose must be given if the 4th dose was administered before the 4th birthday.
IPV/Polio 3 doses are required. A 4th dose must be given if the 3rd dose was administered before the 4th birthday.

MMR

(Measles Mumps Rubella)

2 doses are required. Ideally, the 1st dose is after the 1st birthday and the 2nd dose is after the 4th birthday.
Hepatitis A 2 doses are required and must follow the ACIP schedule.
Hepatitis B 3 doses are required and must follow the ACIP schedule.

Varicella

(Chicken Pox)

2 doses are required. Ideally, the 1st dose is after the 1st birthday and the 2nd dose is after the 4th birthday. If students have had the chickenpox disease, the vaccination is not required.

TDAP

(Tetanus Diphtheria Pertussis)

1 dose is required for all students entering the 7th grade and all new students in grades 8-12

MCV4

(Meningococcal Meningitis)

1 dose is required for all students entering the 7 th grade and all new students in grades 8-12
